Study Books with Patients .... you will learn this way

"To study the phenomena of disease without books is to sail an uncharted sea, while to study books without patients is not to go to sea at all." - Sir William Osler

I am hearing about the procedure "Exchange Transfusion" since final year MBBS ...... i have read this in books for so many times ........ still after reading in detail i had not idea how it all goes in real. Yesterday, i not only saw this practically but i also assisted my senior Doctor in Exchange Transfusion of a 5 year old baby boy, whose bilirubin was 33 mg/dl.

My senior says, stand at bedside, open your books and then examine the patient. I have seen my seniors ... opening his/her books while managing the patient of DKA (Diabetic Ketoacidosis) ..... There is no harm in opening a book in front of patients ..... you will learn this way.

I'll share few more things and my experience about Exchange Transfusion in my coming posts .... see you .... tata

What is SGPT and Why my SGPT levels are high?

Yesterday, i recieved a msg on facebook from someone (not Friend) ........ he wanted to know that what is SGPT and why his SGPT blood levels are bit high ...... his LFT report was attached with the msg ..... showing disturbance in SGPT levels .... that was 51 U/L (normal: upto 41).

I dont know whether he was worried over his raised SGPT or not, but this was clear from his report that some thing is surely wrong with his body .... lets see whats could be the possible reason for his high SGPTWhat is SGPT? its an Enzyme, mainly contained in Liver Cells ..... when the liver is injured, liver cells release SGPT (Serum glutamic-pyruvic transaminase) or ALT (Alanine Transaminase) in blood (circulation), and raising its level in blood ...... and signals that something is wrong with YOUR LIVER, thus its an indicator of your Liver's Status.

What are Enzymes? their Functions? Enzymes are nothing but Protiens, present throughout the body, each having their Unique function. They help to accelerate the necessary chemical reactions in our body.

I dont know, who adviced him this Investigation and why ...... so lets discuss generally, that what are the symptoms in which you should go for this Investigation (but on your Doctor's Request). It is ordered when you have liver disorder symptoms like abdominal swelling .. with or without pain, jaundice (yellow discolouration), itching (pruritis), nausea, vomiting, loss of appetite, weakness, fatigue, dark urine or light coloured stool. It may also be ordered, if the person have known history of Hepatits, or any possible exposure to Hepatitis Virus, if the person is a Drinker, having positive family history of liver diseases, obesity, diabetes or Drug that can cause harm to the Liver. So, the causes of Raised SGPT include: Viral Hepatitis, Alcoholism, Fatty Liver, Obesity, and use of antibiotics.

His further lab reports show that his Anti HCV and HBsAg is Negative, so Viral Hepatits is excluded. He is not a Drinker, and he has not taken any antibiotics recently, so Medication and Alcoholism excluded. Yes, he eats junk food, he is obese, and there must be lack of exercise, so the possible cause of raised SGPT in his case is Fatty Liver.

Medical Advice in his case should be: Loose weight, have low fat diet, do some exercise ... thts it, no need of Liver Tonics and other Medications.

SEHWAG - Smashes 219 to become a New World Record Holder

It was 1997, in chennai, when Pakistan's Saeed Anwar smashed 194 runs in an one-day international against India, to make a record of highest runs by an individual in one-day internationals. In 2009, venue Bulawayo, Zimbabwe's Coventry also smashed 194 (against Bangladesh), but was superior to Anwar's record, because it was a not out innings. In 2010, India's Tendulkar climbed the mount 200, a not out innings against south africa in Gwalior. Remember the words of Brian Lara, which he used to say, when his record was stealed, that, "Records are made to be broken" .... yup ... very true .... On 9th December, 2011 ... Virendar Sehwag (India), steals the record of Tendulkar (200), by smashing 219 in 149 balls against West Indies in Indore. Great ..... Indians are Rocking International Cricket ..... Congratulations to Sehwag, India and Cricket Fans ....

Tips for Residency Interview

1. Look interested

2. SIt up straight

3. Always have some ‘safe’ questions like ‘what do you like best about your hospital’ or ‘what unique opportunities are there here’or “How is the teaching’

4. Never tell a faculty member or interviewer if you are applying in two specialties and the program is a ‘safety’ for you. They cannot handle the ambivalence. I hear it from attendings ‘he has no direction’ . this is AMAZINGLY unfair, but unfortunately, do NOT tell them

5. come early. Leave late. Ask to come for a ‘second look’

6. Smile and make eye contact

7. NEVER NEVER NEVER criticize ANYTHING.

So, each year I get comments like ‘so you are telling applicants to LIE!!!” actually..,,, yes, withold your real feelings. Do NOT be completely honest about things you see as negatives.
If you don’t understand, you are an idiot.

Extra-Ordinary Footage of Uncontacted Tribes - Video Released

Extraordinary aerial footage of uncontacted Amazon tribe released

This unique film shows uncontacted Indians on the Brazil-Peru border in never-seen-before detail. It is the first-ever aerial footage of an uncontacted community. The footage was filmed by the BBC in collaboration with the Brazilian government, for the new BBC 1 ‘Human Planet’ series.

Peru’s President Garcia has publicly suggested uncontacted tribes have been ‘invented’ by ‘environmentalists’ opposed to oil exploration in the Amazon, while another spokesperson compared them to the Loch Ness monster. In fact, there are more than one hundred uncontacted tribes around the world.

Alternative Career Zone for Medical (MBBS) Students

I have personally seen Many Doctors, who have left this field after MBBS, some did this in Frustration and some were in hurry for a Good Job and Good Money. In Pakistan, many MBBS doctors take the exam of CSS (Competitive Exam - Civil Superior Services) after completing MBBS, and join Civil Services. Its a Federal Government Job of BPS Grade 17, Early promotion chances are good in this job and pay scale usually starts form 25 to 30,000. After MBBS u can do MBA also to join Pharmaceutical Companies, if you are smart enough you can join these Drug Companies without doing MBA. The Pay Scale in these companies start from 20 to 30,000 and goes up with the experience. There are many MBBS doctors who dont want to interact with the patients, but they want to work in hospitals, they can do MBA in Hospital Administration, and can join Hospitals with Great Pay. Another option is Certification/Diploma in Clinical Research after MBBS, and join Clinical Research Institutes. You can join Army as a Captain after clearing their Exams. Other than CSS, there are other competitive exams also like PCS, Commission Exam for Lecturer Ship. Biotechnology and Bio-Informatics are two other options after MBBS. There must be lot other options also, but i am not familiar with them yet ......

Summarizing:

  • CSS

  • MBA (Pharmaceuticals)

  • MBA (Hospital Administration)

  • CLINICAL RESEARCH

  • ARMY

  • PCS

  • LECTURER SHIP

  • BIO-TECHNOLOGY

  • BIO-INFORMATICS
